Immunizations: Stop sickness before it starts


Immunization is one of the best ways you can protect yourself and your children against infectious disease. By stimulating your body’s natural resistance to disease — thereby creating immunity — vaccines are your first line of defense against the likes of polio, measles, mumps, rubella, influenza, tetanus, diphtheria and pertussis.


“Were it not for the widespread use of vaccines in the U.S., a far greater number of deaths would occur during childhood and many more people would be living with chronic and often crippling after effects of disease,” says Mayo Clinic Health System pediatrician Joel Cassingham, M.D.


The immediate result of immunization is the prevention of dangerous, potentially life-threatening infectious illnesses. The long-term goal of an immunization program is the complete eradication of a disease. Smallpox, a devastating illness that killed 30 percent of those infected and left the others with disfiguring scars, was completely eradicated through immunization in 1979. Measles cases in the U.S. have dropped by 98 percent, and diphtheria, a condition that caused more than 15,000 deaths in 1921, now is only reported in two or three cases every year because of immunization.


Every vaccine is tested and must meet strict safety standards set by the Food and Drug Administration. This can take up to 10 years and even after being opened to the public, every immunization is monitored by the Centers for Disease Control and Prevention.

Spinal manipulation techniques differ among practitioners


Chiropractors commonly perform spinal adjustments, also known as spinal manipulation, but osteopathic doctors, physical therapists and medical doctors may provide the service, as well.


Specific techniques may differ, but in general, during spinal manipulation, the practitioner applies a controlled amount of force to a spinal joint, either manually or using a device. Alternatively, a rhythmic force is applied to improve joint motion in your neck, spine and pelvis. The practitioner may use a special table to help perform adjustments.


Evidence indicates that spinal manipulation can help with common headaches, neck pain, and low back pain, especially if the pain is recent and not due to compression of a nerve root.


Spinal manipulation generally is considered to be safe when it’s performed by a trained and licensed practitioner. Common side effects are soreness in the treated area, dizziness or tiredness, which typically clears up within a day or two. Serious side effects are very rare, but may include pinching of spinal nerves, resulting in leg pain and weakness or worsening of a herniated disk. High-velocity neck manipulation rarely has been associated with stroke.


Spinal manipulation generally isn’t recommended if you have severe osteoporosis, cancer in your spine, previous spinal surgery or inflammatory arthritis.

    Is your colon healthy? With serious health issues at stake, ask a doctor, not Google

    Nobody really wants to see a gastroenterologist.


    Colon cancer, constipation and irritable bowels are all potential medical problems most people would like to ignore.


    The answers aren’t on the Internet, says Dr. Neelima Reddy, a gastroenterologist at the Digestive Health Center of Louisiana.


    “If you just Google abdominal pain and it gives you 100 causes, how do you know which one is for you?” she says. “Our job is to figure out which one applies to you. Freaking out will only make it worse.”


    So after they’ve searched the Internet, what do patients ask Reddy? Here are the top five questions (and her answers):


    How do I stop constipation?


    Constipation is the top concern patients ask Reddy about. It can cause stomach pain and bloating, which usually drives patients to the gastroenterologist.


    “People try to put it off to a certain degree,” Reddy says. “They think in this culture a little bit of constipation is normal. None of us gets enough fiber.”


    Constipation often begins because young people don’t want to use the bathroom at school or adults avoid going at work.


    “If you try to keep things in, your rectum just gets used to holding the stool in and it doesn’t want to do it when you want to do it,” Reddy says.


    While medications are available, Reddy usually admonishes patients to eat better and exercise.


    “Whatever is good for your body is good for your colon,” she says.


    A good diet for digestive health means small quantities of red meat and more fruits and vegetables.


    Do I really need a colonoscopy?


    Most patients know they should get their colons checked at 50, but many want to avoid it. In a colonoscopy, a doctor uses a camera mounted on a thin tube to check the inside of a patient’s large intestine — the rectum and the colon. Doctors look for pre-cancerous growths called polyps, which can be removed in order to prevent colon cancer.


    “This is like a wellness check-up,” Reddy says. “You do a mammogram or a pap smear or a prostate exam — all of these are preventative screening things.”


    Is a colonoscopy painful?


    In the past, colonoscopies earned a tough reputation. But as technology advances, cameras have gotten tinier and sedation medicine has improved, making it a painless outpatient procedure, Reddy says.


    “You’re not going to feel anything,” she says. “You come in, take a nap, wake up and you’re done.”


    Some patients worry about the sedation medicine, but most people don’t have any complications from it.


    “It’s the best place to fall asleep and the safest thing to do,” Reddy says. “It puts you to sleep quickly and you can wake up quickly.”


    What are the symptoms of colon cancer?


    “We say the most common symptoms of colon cancer is no symptoms,” Reddy says.


    Abdominal pains, blood in the stool or anemia are all signs of colon cancer, but they can also be symptoms of other medical issues, she says.


    Is colon cancer hereditary?


    Most of the time, it isn’t. About 3 percent of colon cancer is inherited, Reddy says. But if a close family member — a brother, sister or parent — has had colon cancer or has had a colon polyp removed, Reddy insists on having a colonoscopy 10 years ahead of schedule. “If they have a polyp, we pretty much treat it the same as cancer because if you leave it, it can become cancer,” she says.

    The feds say screening for testicular cancer isn't necessary. This health advocate is out to prove ...

    The official advice on whether men should regularly check their testicles for signs of cancer is: no, don’t do it.


    But ever since he discovered a lump at the age of 17, health policy researcher Michael Rovito has been on a one-man mission to change the minds of the medical establishment.


    He began when he was in graduate school by studying how best to communicate with men about the risks and signs of testicular cancer. He’s since published a series of papers arguing how and why doctors should tell their patients to check their testicles regularly. And he’s even started a nonprofit — the Men’s Health Initiative — to push his message.


    Yet, Rovito, now 35 and an assistant professor at the University of Central Florida, has a formidable adversary in his path: the US Preventative Services Task Force, which advises the federal government on health issues.


    For more than a decade, the USPTF has recommended against routine testicular cancer screening, either by self-inspection or clinical examination. The reason? There’s no proof it saves lives. “Screening lacks benefits,” task force chair Dr. Albert Siu, a geriatrician at the Mount Sinai Medical Center in New York, told STAT.


    Because treatment is so effective, it’s not crucial to find testicular cancer right when it emerges. Accidental discovery when a lump is big enough to be obvious is often good enough, according to leading physician groups.


    Without some gold rush of funding, it’s highly unlikely that clinical trials of the kind the USPTF wants to see will be run anytime soon. But Rovito argues that the benefits of self-exams extend to finding testicular problems other than cancer, and that regular screening should not cause men too much anxiety — an often-cited risk of the practice (although the harms haven’t been systematically studied).


    “Let’s just think about this from a different perspective from quantitative numbers and mortality rates, please,” he said.


    Rovito is also going on the offensive against the USPTF with a paper now under review that directly calls on the task force to change its stance on testicular screening from a “D” grade, the strongest recommendation against a practice, to an “I,” the rating for insufficient information.

    Some experts already practice what Rovito is preaching.


    Dr. Joseph Alukal, a urologist at New York University’s Langone Medical Center, said that he’s treated patients who tried to ignore a lump in their testicles. Some waited until the mass was grapefruit-sized to come in.


    “What’s the harm,” he said, “in asking [men] to just say, ‘Look, there’s nothing wrong with checking yourself once in the shower a month. Here’s what you should be feeling for.’”


    Alukal said he routinely disregards the USPTF’s recommendations and examines all of his patients for signs of testicular cancer; he also tells most of his patients to check themselves.


    But critics say there are downsides, too.

    Dr. John Tipton, of the University of Oklahoma-Tulsa, said that family medicine doctors like himself typically only have a few minutes with each patient, and doing a testicular examination for each one would take precious time away from other services.


    “Everything that you add on detracts from something important,” Tipton said.


    And for such a rare and treatable disease — only about 9,000 American men will develop testicular cancer this year, and fewer than 400 will die from the disease — Elizabeth Platz, a cancer epidemiologist at Johns Hopkins University, questions whether encouraging screening is a good use of public health resources.


    “Why would we just do something if we don’t know there’s evidence for it?” she said.

    Platz said she remembers when breast self-exams were thought to help women detect tumors early and was encouraged even in college-aged women. Then, a randomized controlled trial came out showing no benefit on breast cancer mortality.


    “Women got very upset about that,” she said. “The evidence changed, so the guideline changed.”


    She said the same principle applies to testicular cancer. While it may seem intuitively smart to have men check themselves, the evidence remains elusive.


    Still, Rovito thinks educating men about risks to their health is worthwhile.


    He recalled a time in sixth grade when the girls in his class were pulled out to learn about menstruation. The boys “had to stay there and play tic-tac-toe,” he said, while the girls “went and learned about their bodies, which I’m kind of jealous about.”


    When he discovered a lump on his testicle in high school, he didn’t know what to do. He waited for it to go away, but it didn’t. “I sat there for a couple months thinking, ‘Oh my god, I’m going to die.’”


    When Rovito finally went to the doctor, it turned out he didn’t have cancer. Instead, the lump was an enlarged vein in the scrotum.


    Rovito now wants other men to be spared the ignorance and fear he felt at the time.


    “If I ever have a chance to educate other guys on not just testicular health but other issues, just to get them more comfortable with bringing up the issues,” he said, “then I’ll consider that a happy life for me.”

    GENDERING THE BODY IN TRADITIONAL MEDICINE: Rethinking Gender, Sexuality, and Sexual Health in Non-Biomedical Discourses and Practices


    Studies of how gendered bodies are conceptualized and acted upon within non-biomedical traditions predominantly focus on male and/or female bodies. Scholars of what is often glossed as alternative or traditional medicine look, for example, at differences of therapeutic practices of male and female healers, the uses of ‘traditional’ and ‘herbal’ beauty and sexual health products, non-biomedical approaches to women’s reproductive health, or the circulation of therapeutic logics about semen retention, virility, and masturbation. Yet, rarely do such studies move beyond male/female bodies to look at the particular epistemological and social understandings of gender and sexuality through which such bodies are ‘treated.’ How do traditional medical practices vary not only in the ways they conceptualize genders but also understand ‘sexuality’ and ‘normality’? How do assumptions about heterosexuality and heteronormativity in various non-biomedical systems differ from one other or from those in biomedical modalities? How do practitioners of alternative/traditional medicine discuss, ‘treat,’ disregard, or accommodate gender variant and queer bodies? What biological, moral, social and political arguments are involved in discourses about heterosexual ‘health’ and homosexuality ‘cures’?


    The challenge of answering these questions lies in the fact that the current literature on Ayurveda, Traditional Chinese Medicine, homeopathy, jamu, sowa-rigpa, Unani, Yoga, and other therapeutic traditions offers little ethnographic and theoretical material on queer bodies. Therefore, in this panel we propose to explore the normative moral discourses and practices of gender and sexuality within non-biomedical traditions. Moving away from the studies, initiated by Foucault (1985), on biomedico-moral perspectives on non-normative sexuality, we invite papers that examine how different therapeutic traditions understand relationships between morality, sexuality, and health with regard to gendered bodies. What kinds of ‘moral symptomatology’ (Zhang 2007) and ‘biomorality’ (Alter 2000; 2011) are implicated in health discourses regarding normative and queer bodies? In addition to theoretical explorations, we invite panelists who will engage with rich ethnographic data. How do we make sense of the claims by popular yoga gurus in India that yoga can “cure” homosexuality? How do Chinese medicine conceptualize (and/or “medicalize”) ideal gendered bodies and sexual deviance? How is it that Ayurveda is deployed in discourses about gendered morality and sexual moderation, while Ayurvedic drug companies advertise drugs for boosting sexual performance? How do government authorities and herbal/traditional pharmaceutical industries define and seek to normalize gendered bodies and sexual practices? Are there economic, ideological, nationalist implications? How can these questions and phenomena shed light on non-biomedical approaches to ‘sexual health’? Finally, how can they contribute to our understanding of predominant biomedical discourses on gendered bodies and practices?


    This panel seeks to draw together scholars in diverse areas, including gender and sexuality studies, medical anthropology, body and embodiment, medical pluralism, political anthropology, and anthropologies of morality and religion. Papers that explore the outlined topics in diverse regional and cultural contexts are welcome.
