Access to affordable birth control — just what the doctor ordered

As a doctor and a researcher, I spend my days working with women in St. Louis as they navigate some of the most important decisions of their lives, including their reproductive decisions. I see the impact of political decisions — decisions that are disconnected from the lived experiences of women — in my daily work as I talk to women about finding the best contraceptive method for them. That is why I am so concerned about the Zubik v. Burwell case currently at the Supreme Court.

Birth control gives women the agency to determine if and when to have children. But as a doctor who works with women every day, I know that it provides women with more than that. Women also need contraceptive care for other medical reasons, including preventing ovarian or uterine cancers and regulating painful periods. That’s why the Affordable Care Act covers contraceptive care with no co-pay. Birth control is fundamental basic health care that every woman — regardless of where she works — should have access to.

Decisions about reproductive care are not just health decisions; they are economic as well. An IUD can cost up to $1,000. That’s more than a minimum wage worker would earn in an entire month. Without insurance, an IUD would be a totally inaccessible option for those women.

In fact, nearly one in three women said they would change their contraception method if cost were not an issue. One of the greatest contributions the ACA’s contraception benefit has offered to women is the ability to remove cost from the equation when determining the best method of contraception for them so they can focus on their health and wellness without exception.

I have seen firsthand the benefits of providing women with the contraceptive method of their choice at no cost — which is what the ACA contraceptive benefit does. My colleagues and I recently concluded a multiyear study on the impact of long-acting reversible contraception on decreasing rates of unintended pregnancy. We offered over 9,000 St. Louis women the reversible contraception method of their choice at no cost, along with education about the different methods. The results were not surprising. With education and access, most women chose long-acting reversible contraceptive methods like the IUD or implant, and ultimately demonstrated a significant reduction in the rate of unintended pregnancy.

The findings of our study, the Contraceptive CHOICE Project, showed that user-independent methods of birth control are more effective than others. Effective use of an IUD or implant is not reliant on a woman’s memory, or a trip to the pharmacy or a visit to the doctor, and therefore does a better job at preventing unintended pregnancies. We found that easing access to these methods of contraception results in a decrease of unintended pregnancies, which in turn means that women have control over their lives and better social, financial, economic and educational outcomes.

It is clear that women succeed when they are able to access contraception with no obstacle. And yet, the case at the Supreme Court threatens to reintroduce obstacles as women seek to access contraceptive care. The Supreme Court will soon hear oral arguments in Zubik v. Burwell, a case that would dramatically and seismically expand the ability of people to use their religion to harm a woman’s capacity to make smart medical decisions for herself.

The Zubik case follows on the heels of Burwell v. Hobby Lobby Stores Inc., which came to the conclusion that closely held religious for-profit employers could refuse to provide contraception coverage required by the ACA. However, in its decision, the court pointed to an “accommodation” offered to religious nonprofits, which allows them to opt out of providing contraception coverage by filling out a one-page form. After the employer opts out, the insurance company provides contraception coverage directly to the employees. The court recognized that this accommodation ensures that women would still get the health care they need as promised under federal law. After Hobby Lobby, the federal government extended the opt-out to closely held for-profit companies with religious objections to providing contraception.

But in the case now before the court, religious nonprofit organizations claim that their religion is burdened by the solution that the Supreme Court pointed to two years ago in Hobby Lobby.

We need to recognize these lawsuits for what they are: continued attacks on women’s access to contraception. Religious employers’ attempts to prevent women from accessing the health care they need have real world implications that play out every day, and will have a direct effect on women’s health and lives.

This case threatens to take choice away from women and make the most effective methods of contraception inaccessible, simply based on where they work. As a doctor, I know that’s not how these decisions should be made. I hope the Supreme Court will agree.

Minnesota Vikings Partner to Expand Access to Women's Health

In order to spread awareness and support for Domestic Violence Awareness Month, Matter partnered with the Minnesota Vikings to collect and distribute women’s health supplies to several local women’s shelters. From the collection of products such as toothbrushes and toothpaste, shampoo and conditioner, feminine hygiene products and grocery store gift cards at the Vikings vs Chief’s game on October 26^th, to a distribution supported by Vikings Cheerleaders and Offensive Tackle Matt Kalil, Matter and the Vikings were able to distribute nearly $200,000 in products to partner organizations who work directly with women in the Twin Cities community.

“We have over eighty member programs in the state of Minnesota that are serving victims of violence and there is a constant need for supplies,” says Katie Kramer, of the Minnesota Collation or Battered Women, one of the recipient organizations of the supplies. “This makes a huge difference and a huge impact to the folks that are doing this work and the women they are serving.”

Products were donated by fans at the October Vikings game and from local businesses like Aveda and Apothecary Products. MATTERboxes, Matter’s healthy food boxes, were also provided to the shelters to expand access to healthy food.

“This is actually just the beginning. We’re going to be doing multiple things with Matter and finding ways that on their behalf and on our behalf we can really go out to the community and the world and do things that Matter.”

Global challenges to women's healthcare access

Around forty students gathered on February 16 at a panel entitled “Women’s Access to Healthcare: Policies and Issues,” to discuss current problems inhibiting women’s access to care in Canada and around the world.

Hosted by the McGill chapters of the Foundation for International Medical Relief of Children (FIMRC) and of the Student Association for Medical Aid (SAMA), three speakers – Zoua Vang, Amm Quamruzzaman, and Simona Bignami – were invited to shed light on the problems currently affecting women’s access to healthcare around the globe, and the effect of policies on these issues. The panel discussion included talks from each speaker followed by audience questions.

Zoua Vang, an assistant professor of sociology at McGill, primarily discussed the role of racial and ethnic determinants in infant and maternal health in the U.S. and Canada. In her talk, Vang stressed that socioeconomic status is a factor that majorly affects the study of access itself.
“You can’t treat all women as this monolithic group,” said Vang. In her talk, she examined the ways in which racial, socioeconomic, and ethnic differences related to premature birth rates, which can vary significantly across populations.

Vang demonstrated that among Asian women, there are discrepancies in the health results of women from different countries. While aggregate numbers show that white and Asian women have similar premature birth rates, there are many ethnic variations at play. By dissecting this larger categorization, “we can see for a lot of these Southeast Asian groups like Hmong, Cambodian, Filipino, Lao, and Thai, their rates of pre-term birth are almost twice that of non-Hispanic whites,” said Vang.

“You can’t treat all women as this monolithic group.”

Vang’s discussion also touched on the difficulties facing immigrant women in the U.S., where there is no universal healthcare.

Quamruzzaman, a PhD candidate in sociology at McGill, spoke about political agency and women’s access to healthcare, specifically addressing cases in the Global South.

“There are physical facilities, but access to those facilities [is] restricted, especially to women, those who are poor, and minority groups,” Quamruzzaman said.

Furthermore, Quamruzzaman identified corruption as a serious problem facing healthcare systems in nations in the Global South. “Many who go to public clinics have to pay a bribe,” he explained.

Quamruzzaman also noted how access to healthcare for women is influenced by systems of governance. “Women’s political participation is significantly linked with infant mortality rates. […] It is imperative to improve their quality of governance by improving their healthcare service prospects.”

Bignami, an associate professor of demography at Université de Montréal, spoke about the struggle of collecting medical data in certain countries.

Bignami pointed out that a great deal of the data presented by Vang is not available for Sub-Saharan African countries.
“For Sub-Saharan Africa, we are limited to [using] data from surveys,” Bignami noted. “Everything else that we know is biased insofar that people told us what they thought happened or what they thought we wanted to hear happened.”

Rebecca Kahn, a U2 Cognitive Science student, stated that she attended the panel because she is “interested in healthcare and its shortcomings in biomedicine, Westernized healthcare especially, and how this affects marginalized groups like women and racial minorities.”

“The people running healthcare systems are white men who don’t know what women and cultural minorities need,” she added.

Jessica Petrov, a graduate student in physiology, thought that “it was very informative – they seemed very well-versed on their topics.” She also added that she would have appreciated “more background information for people who don’t know much about the socioeconomic factors.”

Another Physiology student in attendance, Laurence Carrier, expressed the opinion that the panel discussion lacked cohesion, saying that “they were very fragmented, and I would have liked to see a theme.”

While some attendees thought that the panel was unstructured and lacked focus, and neglected certain issues like trans women’s access to healthcare, others still found it helpful.

“I thought they had some really interesting points that I hadn’t heard of before,” said Kahn.
